Who is responsible for costs and relocation?

I have been renting a condo for almost 7 years. I have found small cracks in the bathroom and kitchen where mold seems to grow rapidly. I have used every cleaner possible to keep these areas clean. Black mold grows within days of my cleaning the area. Three years ago the water heater in the unit above me broke and flooded down into my apartment. That menas it was in the ceiling and walls. Recently we have been having a lot of rain. I noticed that the baseboard in my bedroom was damp. After one heavy rain the carpet in my bedroom was soaking wet and really stinking. I rented a carpet cleaner and wet vacuum to clean it. It still stinks. Today I had a friend move my couch so I could vacuum behind it. I am 61 years old with a bad back so it doesn’t get done as often as I would like. I found damp base boards and damp carpet in my livingroom. The order is terrible. The baseboards have started to rot. I have had a terrible cough and sore throat for quite a while now, accompanied with bad headaches. It clears up a lot when I am not at home for a few days. The doctor told me that he doesn’t know what is causing it and maybe I should move to another location.
My landlord is famous for never returning security deposits or cleaning deposits. The condos are over 50 years old. She owns 6 units that she rents out. When I contact her I know she will tell me that I have to pay for any and all costs. If I tell her that I have to move out because of my health problems from this she will not pay for anything. Should she be responsible for relocation costs? I can’t afford the costs to move. I am on a limited income. Who is responsible?
