Need some advice on what to do..

I am in the process of moving out of a place me and my family have been in for 7 years now. The landlord owns many other properties that seem to be more important than the ones located here. I have been trying to get some things fixed now for over a year and a half and I just keep getting blown off.

The floors are all soft and are about to fall out of the place (some of the floor vents are loose because of this). The lenolium in the kitchen is coming up (I caught my foot and fell) I looked and there is old lenolium under the newer lenoilum. The windows leak so the wood in around the windows are rotting right out. 
There are leaks in the roof. I had one over my bed and there is still a huge hole there after over a year, they have never attempted to fix it.
There are places in the wall under where the window leaked and you could put your hand through them.
There is black mold all over the place and we have a major mouse problem because they never sealed any of the holes around the pipes. I have tried everything to get rid of them and they refuse to help us.

So after putting up with this long enough. And the landlord keeps raising the rent even though he NEVER fixes anything. We are moving at the end of the month I am tired of living like this, and tired of me and my kids being sick all of the time from this. 

We would like to know how to protect ourselves and is there someone like the housing authority or something like that who could walk through and document this stuff. 
Because we know the landlord will try to get the repairs money for this stuff from us. Even though he is at fault for never fixing anything. 
There are a few other places in here with all of the same issues as we have.

Hi Mel! Sorry to hear about your problem on repairs. Do you have any pictures of the condition of the rental unit before you moved in? Your landlord cannot deduct, withhold your security deposit and charge you for repairs if the damages made on the rental unit is normal wear and tear. If your landlord insists that you pay for the repairs that is already present on the rental unit then you can file a complaint against your landlord. You can file your complaint with the RPA.
